Direct Billing
Available for Canada Life, Manulife, Desjardins, Industrial Alliance, Medavie Blue Cross, Green Shield Canada, Empire Life, and more
If you’d like to have have services directly billed to your insurance please fill out this consent form.
Most major insurance plans cover dietitian services – check your benefits booklet or contact your insurance to confirm your coverage.
For other insurance plans that do not support direct billing, a receipt will be provided for you to submit directly.
